Postinflammatory hyperpigmentation (PIH) is usually the result of nodular acne lesions. These lesions often leave behind an inflamed darkened mark after the original acne lesion has resolved. This inflammation stimulates specialized pigment-producing skin cells (known as melanocytes) to produce more melanin pigment which leads to the skin's darkened appearance. People with darker skin color are more frequently affected by this condition. Pigmented scar is a common term used for PIH, but is misleading as it suggests the color change is permanent. Often, PIH can be prevented by avoiding any aggravation of the nodule, and can fade with time. However, untreated PIH can last for months, years, or even be permanent if deeper layers of skin are affected. Even minimal skin exposure to the sun's ultraviolet rays can sustain hyperpigmentation. Daily use of SPF 15 or higher sunscreen can minimize such a risk.

And though none of the experts we spoke to specifically called this out, writer Frankie May Wong declared this salicylic-acid-enhanced Medik8 gel as her cystic-acne Holy Grail. She uses it sparingly to avoid it losing its efficacy, but thanks its blend of acne-fighting ingredients for her clear skin and the lack of scarring thereafter: “Another reason the Beta Gel seemed to work miracles could’ve been the combination of dioic acid (a rarer ingredient that otherwise only appears in the pricey but raved-about SkinCeuticals Blemish + Age Defense serum), azelaic acid, and niacinamide.”

But the milder cases can benefit from some topical over-the-counter treatments, too: “OTC options should be limited to when you have only one or two cysts,” though, says dermatologist Noelani Gonzalez, the director of cosmetic dermatology at Mount Sinai West. “Otherwise you should go see your dermatologist sooner than later to avoid any scarring.” Here, we spoke with three dermatologists to hear more about the most effective cystic-acne treatments to use at home.
There are several low-level light devices designed as at-home acne remedies on the market—but do they really work? Some, like the Zeno electronic "zit-zapper" are FDA-approved as acne remedies, but reviews with these products are typically mixed. Even the best acne treatment won't work for everyone, as the severity of the acne, types of acne and quality of the device are all factors. Ask your dermatologist for a recommendation if you're considering purchasing an at-home light device to treat your acne.
To many parents’ dismay, their beautiful newborn’s face breaks out with red bumps at around 3 to 4 weeks of age. This is called baby acne. It tends to occur at about the same age as the baby’s peak gas production and fussiness. How attractive! (This all coincides with parents’ maximum sleep deprivation.) Parents are often quite concerned both about how these bumps look and about their significance.
Chemical peels can be used to reduce the appearance of acne scars. Mild peels include those using glycolic acid, lactic acid, salicylic acid, Jessner's solution, or a lower concentrations (20%) of trichloroacetic acid. These peels only affect the epidermal layer of the skin and can be useful in the treatment of superficial acne scars as well as skin pigmentation changes from inflammatory acne. Higher concentrations of trichloroacetic acid (30–40%) are considered to be medium-strength peels and affect skin as deep as the papillary dermis. Formulations of trichloroacetic acid concentrated to 50% or more are considered to be deep chemical peels. Medium-strength and deep-strength chemical peels are more effective for deeper atrophic scars, but are more likely to cause side effects such as skin pigmentation changes, infection, and small white superficial cysts known as milia.
You’ve probably heard of the benefits of retinoid creams for anti-aging, but vitamin A is also efficient at clearing up acne. “[Retinoids] cause skin cells to turn over at a faster rate, decrease oil production, and help skin exfoliate,” board-certified dermatologist Rita Linkner, M.D., tells SELF. Another benefit: Acne is inflammation, and retinoids are anti-inflammatory.
You've probably seen the Proactiv clear skin system advertised on television at some point over the last several years, but does it work? Proactiv is one of the better acne remedies out there for mild cases of inflammatory and noninflammatory acne, hormonal acne and adult acne. The starter kit comes with a gentle benzoyl peroxide exfoliating cleanser, a glycolic acid pore cleanser, and a benzoyl peroxide repairing treatment.
Antibiotics are an acne treatment used to kill acne-causing bacteria. They may be applied directly on the skin (topical) or taken by mouth (oral). Topical antibiotics kill bacteria in the upper portion of your pores, while oral antibiotics can reach to the lower depths of the pores. Antibiotics used for acne treatment include clindamycin or tetracyclines like doxycycline or minocycline. These antibiotics are the most effective for treating acne because they both kill bacteria and act as anti-inflammatory agents to calm down the skin.
Even better than one stand-alone cream is a multipronged combination of acne treatments. “A good place to start is to use a combination of salicylic acid, benzoyl peroxide, and a topical retinoid,” says Joshua Zeichner, the director of cosmetic and clinical research in dermatology at Mount Sinai Hospital. “Salicylic acid helps remove excess oil from the surface of the skin and exfoliate the dead cells, while benzoyl peroxide kills acne-causing bacteria and helps reduce inflammation.”

People who escaped their teen years almost pimple-free may develop persistent adult-onset acne as they get older. Despite the normal increase in androgen levels during puberty, some doctors believe that flare-ups of acne have less to do with androgen levels than with how a person's skin responds to an increase in sebum production or to the bacteria that causes acne. The bacteria Propionibacterium acnes occurs naturally in healthy hair follicles. If too many of them accumulate in plugged follicles, they may secrete enzymes that break down sebum and cause inflammation. Some people are simply more sensitive than others to this reaction. Sebum levels that might cause a pimple or two in one person may result in widespread outbreaks -- or even acute cystic acne -- in another person.
